Job description

Nurse Practitioner Job Overview
We are seeking a dedicated and skilled Nurse Practitioner to join our healthcare team. The ideal candidate will provide high-quality patient care, perform assessments, and develop treatment plans in various medical settings. This role requires a strong background in clinical practice, excellent communication skills, and the ability to work collaboratively with other healthcare professionals.

Responsibilities

  • Primary care office, home visits, Assisted Living facility visits.
  • Conduct comprehensive physical examinations and patient assessments.
  • Develop and implement individualized treatment plans based on patient needs.
  • Provide patient education on health management and disease prevention.
  • Collaborate with physicians and other healthcare providers to ensure coordinated care.
  • Maintain accurate medical documentation in compliance with HIPAA regulations.
  • Manage acute and chronic conditions across various specialties including internal medicine, geriatrics, and urgent care.

Skills
The successful candidate will possess a combination of the following skills:

  • Proficiency in EMR/EHR systems such as Tebra.
  • New grads are welcome to apply
  • Knowledge of medical terminology, ICD coding (ICD-9 & ICD-10), CPT coding
  • Ability to perform diagnostic evaluations and manage complex patient cases including those requiring telehealth services or home care support.
