Job description

Resource Logistics Inc is seeking a travel nurse RN Case Management for a travel nursing job in Burbank, California.

Job Title: Registered Nurse – Case Manager
Location: Burbank, CA
Contract Length: 13 Weeks (High Possibility of Extension)
Schedule: Day Shift – 5x8-Hour (08:00 AM – 04:30 PM)Key Responsibilities:
Perform patient assessments, care coordination, and discharge planning for acute care patients.
Determine medical necessity using evidence-based guidelines and regulatory standards.
Coordinate post-acute care services, DME, and community resources to ensure smooth patient transitions.
Develop and implement individualized plans of care in collaboration with physicians and interdisciplinary teams.
Maintain accurate computerized documentation using Epic EMR.
Ensure compliance with CMS, HIPAA, DRG, CPT coding, and Department of Health regulations.
Manage a caseload of up to 25 patients while maintaining high-quality patient care standards.
Requirements:
Active RN License
BLS Certification
Minimum of 2 years of acute care Case Management experience performing Care Coordination and Discharge Planning
Experience in an acute hospital setting required
Epic EMR experience required
Travel experience preferred (first-time travelers will be considered)
Experience with HMOs, IPAs, and utilization review processes preferred
Experience working with ICU, Med Surg, and PCU/Stepdown patient populations preferred

Resource Logistics Inc Job ID #3293902. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
