Position Overview

Renewed Perspectives Wellness is searching for a dedicated Licensed Clinical Social Worker (LCSW) to join our multidisciplinary team in providing exceptional mental health care within a remote setting focused on hospice and end-of-life care. This full-time role allows licensed professionals to deliver compassionate support and clinical expertise to patients and families facing challenging emotional and psychological transitions.

Work Environment

This position is performed remotely, allowing for flexibility while ensuring a collaborative spirit remains intact through regular virtual meetings and team check-ins. Our hospice center acts as a critical support system for families during difficult times, enabling you to work closely with other professionals while maintaining the comfort and privacy necessary for sensitive patient interactions.

Key Responsibilities

• Conduct thorough clinical assessments to gather crucial data regarding patients' mental health statuses and needs.
• Develop and implement individualized treatment plans utilizing evidence-based practices tailored to patient circumstances.
• Utilize various therapeutic modalities, including solution-focused therapy, cognitive-behavioral therapy, and crisis intervention strategies.
• Collaborate effectively with a diverse team of healthcare professionals, including nurses, physicians, and chaplains, in the treatment and care of patients.
• Maintain meticulous and timely clinical documentation using our electronic records system to ensure compliance and facilitate continuity of care.
• Provide educational resources and therapeutic support to patients and their families, equipping them with tools to cope with emotional and psychological challenges.
• Engage in continued education to remain attuned to the latest research in mental health, adapting practices accordingly and contributing to best practices within the team.

Patient Population / Client Focus

As an LCSW at Renewed Perspectives Wellness, you will work with a varied patient population that includes adults and their families navigating the complexities of serious illness and end-of-life care. Your role is integral to helping individuals cope with grief, loss, anxiety, and depression associated with these experiences, enabling them to enhance their quality of life during challenging times.

Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of New Jersey is mandatory.
• A strong understanding of solution-oriented therapeutic techniques and approaches.
• Exceptional communication skills, both written and verbal, coupled with a problem-solving mindset to effectively support patients' needs.

Preferred Experience

• Experience working in hospice or palliative care settings is highly desirable.
• Familiarity with electronic health records (EHR) systems.
• Previous experience in a multidisciplinary team environment is a plus.
