Work Environment

This position is primarily onsite, allowing you to work directly within patients' homes across Waldwick and surrounding areas. Key Responsibilities
  • Conduct thorough evaluations to assess the physical and functional limitations of your patients in their home environments.
  • Develop and implement tailored treatment plans that focus on achieving specific rehabilitation goals.
  • Provide hands-on therapeutic interventions, including exercises to improve strength, coordination, and mobility.
  • Continuously monitor patient progress and adjust treatment plans as needed, documenting every change accurately in our electronic health record (EHR) system.
  • Educate patients and their families on exercises, safety practices, and lifestyle adjustments to promote optimal recovery and well-being.
  • Actively participate in team meetings and case reviews to discuss patient progress and collaborate on care strategies.
  • Stay updated with the latest practices and developments in physical therapy by attending workshops and continuing education courses.
Patient Population / Client Focus

As a Home Health Physical Therapist, you will primarily work with geriatric patients, individuals recovering from surgery, and those suffering from conditions such as stroke, neurological disorders, or orthopedic injuries. Your role is crucial in facilitating their rehabilitation and enhancing their mobility, helping them regain confidence in their daily activities.

Qualifications
  • Bachelor's or Master's degree in Physical Therapy from an accredited institution.
  • Current state licensure to practice physical therapy in New Jersey.
  • Dedication to ongoing professional development and adherence to best practices in patient care.
Preferred Experience
  • A minimum of 2 years of experience in physical therapy, particularly in a home health or geriatric setting.
  • Prior experience with various therapeutic modalities and techniques, including manual therapy.

Compensation and Benefits
