About Company:

At Camino Real Community Services, we have built a team of talented, caring, experienced care providers to serve the local community. 

About the Role:

Under the supervision of the Community Support Supervisor, this position works as a Provider Coordinator to a caseload of individuals receiving waiver services in Wilson and Atascosa Counties. Duties include monitoring to ensure that the individual is receiving services as outlined in their Person Directed Plan and Individual Plan of Care. Develops Implementation Plans and reviews all services with the individual on a monthly basis. This position develops and maintains a system of tracking to ensure that the above mentioned activities occur in a timely manner. This position also provides care, training, transportation and employment services. Staff develops strategies to coincide with the individuals Person-Directed Plan and completes appropriate documentation. The Provider Coordinator must train in self-help skills, pre-vocational skills, and Special Therapy and Behavioral techniques, as directed by professional staff. Must be willing to work flexible hours. Must have a Texas Driver’s license, acceptable driving record and dependable transportation. Establishes positive relationships with individuals, families, community members, business community, and staff. Will provide services within a wide range of activities in accordance with requirements of the program. Staff will appropriately maximize revenues by ensuring that all service provision activities are documented correctly and submitted within the agency-specified time frame. Will clearly understand the billing guidelines for the waiver programs and maximize billing through monitoring documentation. Staff will conduct agency business in an honest, fair, professional and integrity-based manner.

Minimum Qualifications:

  • High school diploma or equivalent.
  • 2 years experience in a field related to the duties of this position or an Associate's degree in social, behavioral, or human services
  • Valid driver's license.  Must maintain a clear driving record.

Preferred Qualifications:

  • Bilingual English and Spanish
  • Excellent communication, organizational, and problem-solving skills.

Responsibilities:

  • Document and maintain provider records.
  • Collaborate with internal teams.
  • Assist in the development and implementation of provider service plans.

Skills:

The Provider Coordinator utilizes strong communication skills daily to interact effectively with healthcare providers, parents, patients, and internal teams, ensuring clarity and professionalism in all exchanges. Organizational skills are essential for managing multiple provider schedules, maintaining accurate records, and coordinating complex workflows to support patient care. Problem-solving abilities are applied to resolve provider issues, streamline processes, and address compliance challenges proactively. Proficiency with EHR systems and data management tools enables the Provider Coordinator to maintain up-to-date provider information and support reporting requirements.
