Prominent Talents is seeking an experienced Home Health Clinical Manager (RN) for an established and growing healthcare organization in the Orlando area.

Position Overview
As the Clinical Manager, you will oversee the clinical operations of Skilled Home Health, Personal Care, and Private Duty Nursing services. You'll provide leadership to nursing and caregiving staff, ensure compliance with state and federal regulations, support quality improvement initiatives, and help deliver compassionate, patient-centered care throughout the community.
Key Responsibilities
  • Provide clinical oversight for Skilled Home Health, Personal Care, and Private Duty Nursing services.
  • Supervise Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), Home Health Aides (HHAs), and caregivers.
  • Participate in hiring, onboarding, orientation, competency validation, coaching, and performance evaluations.
  • Develop, implement, and oversee individualized patient care plans in collaboration with physicians and interdisciplinary teams.
  • Ensure patient care complies with physician orders, agency policies, Medicare regulations, and industry best practices.
  • Provide clinical guidance for complex patient cases, documentation, and regulatory compliance.
  • Conduct supervisory visits, competency assessments, chart audits, and quality assurance reviews.
  • Monitor clinical outcomes and implement quality improvement initiatives.
  • Investigate and resolve patient concerns, incident reports, and caregiver issues professionally.
  • Ensure timely and accurate completion of OASIS assessments, Plans of Care, recertifications, discharges, and clinical documentation.
  • Collaborate with Intake, Scheduling, Billing, Human Resources, and leadership teams to support operational excellence.
  • Participate in infection prevention, emergency preparedness, accreditation activities, and internal audits.
  • Educate patients and families regarding treatment plans, disease management, medications, and available community resources.
  • Support referral relationships and timely patient admissions.
  • Participate in an on-call clinical rotation as needed.
Qualifications
  • Current, unrestricted Florida Registered Nurse (RN) license.
  • Associate Degree or Diploma in Nursing required.
  • BSN preferred.
  • Clinical experience in Home Health, Private Duty Nursing, or community-based healthcare.
  • Minimum of two (2) years of leadership or clinical management experience in Home Health or a related setting.
  • Experience overseeing Skilled Home Health, Personal Care, and Private Duty Nursing services is highly preferred.
  • Experience working with Medicare, Medicaid, Managed Care, and commercial insurance.

  • Leadership, staff development, and conflict resolution
  • Microsoft Office Suite
  • Excellent communication, organization, and critical thinking skills
Additional Requirements
  • Current BLS Certification
  • Valid Florida driver's license, reliable transportation, and automobile insurance
  • Ability to travel throughout the service area for supervisory visits and patient assessments
  • Bilingual (English/Spanish) is highly preferred
