Work Environment

This full-time position is based onsite at our modern rehabilitation facility in Springfield. Key Responsibilities
  • Conduct comprehensive evaluations of patients by reviewing pertinent medical histories, performing specialized assessments, and documenting findings and treatment recommendations.

  • Communicate evaluation results and develop personalized treatment plans in collaboration with patients and their families, ensuring that their input plays a critical role in goal-setting.

  • Implement and adapt treatment protocols effectively based on initial evaluations, patient progress, and the evolving healthcare landscape.

  • Engage in interdisciplinary consultations as needed, recognizing changes in patient conditions that may necessitate adjustments in care plans or further medical evaluations.

  • Complete detailed discharge planning and follow-up care. Maintain accurate and timely documentation of patient interactions, treatment plans, progress notes, and discharge summaries, ensuring compliance with regulatory standards.

  • Manage personal productivity effectively, keeping an organized schedule and being punctual for team meetings and patient appointments.

  • Contribute to the continuous development and achievement of departmental goals, ensuring compliance with all relevant governmental policies and accreditation requirements.

  • Pursue ongoing professional development and education, utilizing opportunities for advanced certifications and serving as a clinical instructor for interns and students.

  • Promote a compassionate and efficient customer experience, collecting feedback to enhance service delivery and patient engagement.

  • Involve yourself actively in eliminating barriers to timely access to care and participate in quality improvement initiatives related to service delivery.

Patient Population / Client Focus

As a Physical Therapist at ProActive Therapy Centre, you will work primarily with a diverse population of patients seeking rehabilitation services due to orthopedic injuries, post-surgical recovery, neurological conditions, and chronic pain management. Our therapists provide individualized care that addresses both physical and emotional needs, ensuring an empathetic approach that fosters trust and facilitates healing.

Qualifications
  • Bachelor's degree in Physical Therapy from an accredited institution.

  • High School Diploma or GED is required.

  • State licensure as a Physical Therapist is mandatory for practice in Illinois.

  • Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

  • No prior experience is necessary; however, a genuine desire to learn and grow within the field is essential.

Preferred Experience
  • 1+ years of experience as a Physical Therapist is preferred, but enthusiastic new graduates are encouraged to apply.

  • Experience in outpatient rehabilitation settings is a plus.

Compensation and Benefits
