Duties

• Collaborate with other healthcare professionals to coordinate comprehensive patient care.
• Provide specialized care for senior patients, addressing their unique health needs.

Experience.

• Board certification NCCPA
• Experience in patient care across diverse populations is essential.
• Strong communication skills and the ability to build rapport with patients are crucial for success in this role.
