Key Responsibilities

• Conduct thorough assessments for clients and develop personalized treatment plans that align with their specific needs.
• Provide individual therapy, group counseling, and family support sessions, utilizing evidence-based approaches and therapeutic modalities.
• Monitor client progress, offering feedback and adjusting treatment plans as necessary to optimize outcomes.
• Collaborate closely with a team of mental health professionals, including psychiatrists, psychologists, and nurses, to ensure comprehensive, coordinated care for each client.
• Respond to crises by providing short-term intervention and support in high-stress situations, demonstrating a calm and proactive approach.
• Maintain accurate documentation of client interactions, treatment plans, and progress notes, ensuring compliance with legal and ethical standards.
• Stay abreast of current research, best practices, and emerging trends in mental health to enhance service delivery and clinical effectiveness.

Patient Population / Client Focus

Your primary focus will be on individuals and families navigating the complexities of hospice care, mental health challenges, and other life transitions. You will work with clients facing a range of issues, including grief, anxiety, depression, and substance use disorders. Your empathetic approach and clinical expertise will help clients process their emotions and develop coping strategies, ultimately leading to improved mental health outcomes.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• A valid and current state license to practice clinical social work (LCSW) in Georgia.
• Proficiency in utilizing telehealth technology and virtual communication platforms.
• Exceptional interpersonal and communication skills, with a strong commitment to client confidentiality and trust.

Preferred Experience

• Previous experience working in hospice or palliative care settings, particularly with clients dealing with terminal illness.

• Familiarity with case management systems and electronic health records (EHR) to streamline documentation and tracking of client progress.

Compensation and Benefits
