Work Environment

This role is performed onsite, specifically within patients' homes, allowing for a personalized approach to treatment. Key Responsibilities

• Conduct thorough assessments of patients' physical capabilities and functional limitations, ensuring accurate and comprehensive evaluations.
• Design and implement individualized treatment plans tailored to each patient's specific conditions, goals, and lifestyle.
• Administer various therapeutic modalities, including hands-on techniques, therapeutic exercises, and equipment use, to promote healing and enhance mobility.
• Provide patient education on injury prevention, proper body mechanics, and lifestyle modifications to strain the importance of ongoing self-care.
• Track and document patients' progress accurately throughout their treatment journey, adjusting plans as needed to ensure optimal recovery.
• Collaboratively work with other healthcare professionals, such as physicians and occupational therapists, to coordinate a cohesive treatment approach focused on the patient's overall health.
• Maintain meticulous records of treatment activities and outcomes, ensuring compliance with legal and ethical guidelines.

Patient Population / Client Focus

The patients you will serve at Potential Motion Health encompass a broad demographic with diverse needs. Common conditions addressed include back and neck pain, joint pain from osteoarthritis, sports injuries, post-surgical rehabilitation, balance and gait deficits, and various musculoskeletal disorders. You will play a vital role in improving patients' quality of life through rehabilitative care tailored specifically to their circumstances.

Qualifications

• A Bachelor’s or Master’s degree in Physical Therapy from an accredited educational institution.
• Active state licensure to practice as a Physical Therapist.
• Current board certification by the American Physical Therapy Association (APTA).
• A minimum of one year of clinical experience is required; however, a preference for candidates with more experience will be given.
• Proficiency in modern physical therapy techniques, equipment, and rehabilitation strategies.
• Excellent analytical skills and the ability to develop effective treatment plans based on comprehensive evaluations.
• Exceptional interpersonal and communication skills to foster relationships with patients, families, and other healthcare team members.

Preferred Experience

While a minimum of one year of clinical experience is required, candidates with at least five years of hands-on patient care experience in various settings are preferred. Experience in home care, geriatrics, or working with clinical conditions like post-surgical recovery, arthritis, or neurological disorders is particularly valuable.

Compensation and Benefits

Potential Motion Health offers an attractive annual salary range of $100,000 to $120,000, commensurate with experience and expertise. In addition to competitive salaries, we provide a comprehensive benefits package that includes:
