This full-time role offers an opportunity for a skilled professional eager to make a lasting impact in the field of rehabilitation. Offering guidance and expertise, you will work hands-on with clients to develop individualized treatment plans that optimize their recovery and enhance their quality of life.

About the Organization

Pinnacle Flex Wellness Clinic is a recognized leader in comprehensive home healthcare, deeply committed to revolutionizing the experience of rehabilitation and support for our patients. Our mission is to provide cost-effective and personalized care, delivered in a patient-centered approach that promotes independence and wellbeing. We specialize in homecare services, hospice care, and rehabilitation to fully meet the unique needs of each individual, ensuring that we deliver excellence in every interaction.

Work Environment

This position is onsite at our rehabilitation center located in the vibrant city of San Francisco, a hub for innovation and healthcare. Working closely with an interdisciplinary team of healthcare professionals, you will have access to state-of-the-art facilities and resources, enabling you to effectively deliver superior physical therapy services. Your role will not only involve clinical responsibilities but will also foster a collaborative spirit that enhances team dynamics and client outcomes.

Key Responsibilities
  • Conduct direct physical therapy interventions to alleviate pain, prevent disabilities, and improve function according to established treatment plans.
  • Supervise and mentor Physical Therapy Assistants in implementing precise care strategies tailored to individual patient needs.
  • Assess patient functional levels through diagnostic evaluations to provide comprehensive insights into their physical capabilities.
  • Analyze and recommend environmental changes to clients' homes, facilitating improved independence during rehabilitation.
  • Administer advanced therapeutic techniques, including manual therapy, exercise guidance, and other modalities as per physician prescriptions.
  • Evaluate, fit, and make necessary adjustments to prosthetic and orthotic devices in collaboration with prescribing physicians.
  • Maintain accurate and thorough clinical records including evaluation reports, care plans, and progress notes that align with regulatory standards.
  • Participate actively in interdisciplinary team meetings to contribute to cohesive client treatment plans.
  • Communicate effectively with clients, families, and healthcare teams to discuss treatment goals and progress updates.
  • Complete thorough functional assessments and community resource evaluations to drive patient-centered care.
Patient Population / Client Focus

Your role will predominantly focus on clients recovering from orthopedic injuries, surgeries, or chronic conditions requiring specialized rehabilitation. You will engage with diverse populations, understanding their unique backgrounds and recovery journeys. This personalized approach helps to ensure that interventions are not only clinically relevant but also culturally sensitive, ultimately fostering a trusting relationship with clients and their families.

Qualifications
  • A Doctor of Physical Therapy (DPT) degree from an accredited institution is required.
  • Minimum of 1 year of experience in physical therapy, especially in orthopedic or acute care settings.
  • A current and valid physical therapy license in the state of California.
  • Certification in CPR is essential.
  • A valid driver's license and reliable transportation are required to facilitate home visits.
  • Strong commitment to clinical excellence, patient care quality, and ethical standards.
  • Effective communication and interpersonal skills are critical for fostering relationships with clients and team members.
Preferred Experience
  • Background in working with homecare settings and familiarity with community resource networks.
  • Experience mentoring or training Physical Therapy Assistants or other healthcare support staff.
  • Proficiency in electronic health record systems and familiarity with federal and state regulations governing healthcare practices.
Compensation and Benefits

The position offers a competitive annual salary range of $90,000 to $110,000, commensurate with experience and qualifications. In addition to a robust salary structure, we provide comprehensive benefits including health, dental, and vision coverage, as well as flexible spending accounts. Additionally, our staff enjoy a 401K retirement plan, generous PTO, and company-paid holidays, promoting a healthy work-life balance.
