New Employee Orientation (NEO)
  • All team members attend NEO offered Monday - Friday both onsite and online.
  • Day one includes in-person training at our West Seneca, NY office to introduce our culture and Care Coordination

Hybrid Schedule
  • Care Coordinators are required to attend three onsite days per week during the first 90 days to support on boarding and integration
  • After successfully completing this period, team members may transition to a hybrid schedule for added flexibility. Supervisors may extend the on-site requirement if needed.

Care Coordinator: HYBRID
Person Centered Services Care Coordination Organization is looking for passionate Care Coordinators to work within a health home model serving people with intellectual and development disabilities. The Care Coordinator has an overall responsibility for coordinating and advocating for all aspects of the individual's care including, but not limited to, health and behavioral healthcare, community supports, and other services required to meet the needs of the individual. Care Coordinators use a person-centered, holistic approach, where each person being served has their health care and other service providers collaborate to ensure their wants and needs are met. We're in the business of helping people and always using a strengths-based approach.
Social workers, care managers, and case managers are encouraged to apply!
Position Responsibilities:
  • Develops a comprehensive, person-centered Life Plan with the individual and their circle of support, as well as their entire service provider team.
  • Commits to a respectful, just, and supportive environment for individuals and coworkers aligning with the company's commitment to diversity, equity, and inclusion.
  • Effectively manage a tiered caseload, while tailoring services to individual needs.
  • Completes enrollment and eligibility documentation.
  • Meets with individuals in their homes, physician/provider offices, and other public places in order to conduct assessments and provide services.
  • Work independently from home, as well as reporting to the office at least once per week but may require more depending on business needs.

Requirements:
  • Bachelor's degree with 2 years of relevant experience OR a Licensed Registered Nurse with 2 years relevant experience OR Master's degree with 1 year of relevant experience
    • Relevant experience consists of providing direct care or support, coordination, and/or advocacy for any person(s) on a regular basis. Examples: Direct Support Professional, Teacher, Paraprofessional, Counselor, Social Worker, Care Coordinator, etc.
    • Minimum of a Bachelors degree, or a Licensed RN, is required by OPWDD in NYS
  • Valid driver's license
