Reports to: Director of Nursing Services

Pay Range: $35 to $42 per hour (DOE/DOQ)

Hours per Week: 40 hours

Peninsula Behavioral Health is seeking a compassionate and motivated Population Health Registered Nurse to join our interdisciplinary team. In this role, you'll help individuals with chronic medical and behavioral health conditions improve their overall health, manage symptoms, and achieve greater independence in the community.


What You'll Do

  • Provide chronic condition management and care coordination for clients with complex health needs.
  • Conduct outreach and engage clients in healthcare services.
  • Assess client needs and develop individualized care plans.
  • Educate clients on disease management, medications, and healthy lifestyle practices.
  • Support self-management goals using motivational interviewing techniques.
  • Collaborate with providers, health plans, community partners, and care teams.
  • Assist with transitions of care and population health initiatives.
  • Participate in crisis intervention, outreach services, and occasional home visits as needed.
  • Maintain accurate documentation in the electronic medical record (EMR).


Qualifications


Required:

  • Current Washington State RN or LPN license.
  • Graduate of an accredited nursing program.
  • Valid Washington driver's license, reliable transportation, and proof of insurance.
  • Minimum one year of clinical nursing experience.
  • Ability to pass a background check and pre-employment drug screening.


Preferred:

  • Experience in behavioral health, community health, case management, or population health.
  • Experience working with individuals experiencing chronic mental illness and complex medical needs.
