) Home Health Physical Therapist (PT)
Job Summary:
We are seeking a compassionate and skilled Home Health Physical Therapist to join our team at Pathwell Health in Fairfax, Virginia. As a key member of our interdisciplinary care team, you will provide high-quality physical therapy services to patients in the comfort of their own homes. Our company is committed to delivering personalized care, building trust with our patients, and fostering strong relationships with our community.
Responsibilities:
  • Conduct comprehensive physical therapy assessments and develop personalized treatment plans to achieve patients' goals and improve their quality of life
  • Provide one-on-one physical therapy interventions, including exercises, modalities, and education to patients and their caregivers
  • Collaborate with our interdisciplinary team, including physicians, nurses, and social workers, to ensure comprehensive care plans
  • Maintain accurate and timely documentation of patient progress, treatment plans, and billing information
  • Develop and implement strategies to prevent hospital readmissions and improve patient outcomes
  • Stay up-to-date with the latest evidence-based practices and guidelines in physical therapy

Requirements:
  • Home Health Physical Therapist with a Doctor of Physical Therapy degree from an accredited program
  • Current licensure as a Physical Therapist in the State of Virginia
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Home Health Physical Therapist with a proven track record of providing effective and compassionate care to patients in a home health setting
