Home Health Registered Nurse RN

Job Summary:

Pathwell Health is a trusted, locally-owned and community-focused home health care provider, dedicated to delivering high-quality care to patients in the comfort of their own homes. We are seeking a compassionate and skilled Home Health Registered Nurse RN to join our team of dedicated healthcare professionals.

Responsibilities:

  • Provide patient-centered care to patients in their homes, focusing on their unique needs and goals

  • Conduct initial assessments, develop and implement individualized care plans, and evaluate patient progress

  • Collaborate with interdisciplinary teams, including physicians, therapists, and caregivers, to ensure comprehensive care

  • Administer medications, treatments, and therapies as prescribed by physicians and in accordance with agency policies

  • Document patient information accurately and in a timely manner, ensuring compliance with regulatory requirements

  • Participate in quality improvement initiatives to enhance patient outcomes and agency performance

Requirements:

  • Current RN licensure in the state of California

  • Minimum 1 year of experience in home health or a related field

  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and interdisciplinary teams

  • Ability to lift, move, and transfer patients as needed

  • Reliable transportation and the ability to travel to patients' homes
