Position Summary
The Community Outreach Team Member – Clinical supports Deborah Heart and Lung Center’s RuralCare Connect program through community-based clinical screenings, outreach activities, patient education, and care navigation services throughout Burlington and Ocean Counties. This role will participate in mobile health screenings, outreach events, and preventive care initiatives focused on improving access to care for rural and underserved populations.
The clinical outreach team member will perform basic clinical screening services including blood pressure screenings, glucose testing, cholesterol screenings, and other approved preventive screening activities, while also supporting patient engagement, referrals, and follow-up coordination in collaboration with the RuralCare Connect call center and clinical teams.
Key Responsibilities
Perform community-based preventive health screenings including blood pressure, glucose, cholesterol, pulse oximetry, and other approved screening services
Accurately document, communicate, and provide basic interpretation of screening results in accordance with established clinical protocols
Educate participants regarding screening results, preventive care, and available Deborah services
Facilitate referrals and follow-up coordination through collaboration with the RuralCare Connect call center and clinical teams
Participate in mobile outreach events, health fairs, screenings, and community engagement initiatives
Assist with event setup, patient registration, outreach logistics, and documentation
Support tracking of outreach metrics, screening outcomes, and participation data
Build and maintain positive relationships with community organizations and partner agencies
Ensure compliance with clinical, safety, infection prevention, and documentation standards
Experience
Required: includes current licensure as an LPN or higher in the State of New Jersey and experience in community health, ambulatory care, patient education, preventive screenings, or outreach programs. Strong interpersonal, organizational, and communication skills are required. Experience with community outreach or population health initiatives preferred.
Schedule Requirements
This is a full-time position that requires flexibility to support community outreach events, health screenings, and program initiatives during daytime, evening, and weekend hours as needed. Travel throughout the service area is required.
HOURS: full-time 80 hours per pay period
This position is grant funded and aligned with a specialized program initiative. Ongoing employment is contingent upon continued program funding and organizational need.
The grant approved rate for this position is $26.25 per hour.
