Key Responsibilities
  • Conduct comprehensive assessments that explore clients’ social, emotional, and environmental challenges, utilizing evidence-based tools to inform interventions.
  • Develop individualized service plans focused on clients’ goals, utilizing collaborative strategies to monitor progress and adapt to changing needs.
  • Provide one-on-one counseling and therapeutic support for individual clients and families navigating complex life challenges.
  • Facilitate crisis intervention sessions, conducting risk assessments that prioritize client safety and wellbeing.
  • Network with external community agencies, coordinating services and advocating on behalf of clients to enhance resource accessibility.
  • Engage the community through outreach initiatives that promote Oregon Medical Center’s services, raising awareness about the resources available to those in need.
  • Participate actively in interdisciplinary team meetings and case conferences, ensuring alignment with best practices and continuous professional development.
Patient Population / Client Focus

The LCSW will work closely with clients facing a variety of challenges, including those experiencing significant life transitions, mental health crises, or environmental stressors. Our client population frequently includes individuals affected by chronic illnesses, supportive bereavement needs, and familial complications. Your efforts will play a crucial role in empowering these individuals to navigate their circumstances effectively and rediscover their strength.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • Must hold an active state licensure as a Licensed Clinical Social Worker (LCSW).
  • Strong background in providing social services, ideally in a healthcare or community organization.
  • Knowledgeable about socioeconomic justice issues, community resources, and trauma-informed approaches to care.
  • Excellent oral and written communication skills, with a proven ability to work effectively in culturally diverse settings.
Preferred Experience
  • Prior experience in hospice, palliative care, or related social service settings.
  • Familiarity with electronic health record systems (EHR) and case management software.
  • Experience employing evidence-based practices in social work interventions.
Compensation and Benefits

Oregon Medical Center offers a competitive salary range of USD 60,000 – 100,000 annually, commensurate with experience and qualifications. In addition to salary, we provide a comprehensive benefits package that includes:
