Job description

About the Role
One Home Health is looking for a part-time Physical Therapist Assistant to join our home health team in Valparaiso, Indiana.  
What You'll Do
  • Provide physical therapy services and patient education in the home in accordance with the physician’s Plan of Care
  • Complete visits and documentation accurately and within established timelines and agency standards
  • Monitor patient progress and promptly report changes in condition or schedule to the supervising therapist or nurse
  • Collaborate with the interdisciplinary care team, including participation in care conferences and referrals as appropriate
  • Follow all safety, infection control, and equipment-use protocols to ensure a safe environment for patients and staff
  • Maintain strict patient confidentiality and protect sensitive information at all times
  • Comply with all agency policies, as well as state, federal, and professional standards
  • Participate in ongoing training, staff meetings, and quality improvement activities
  • Perform additional duties as assigned

What You Bring
  • Current license to practice as a Physical Therapist Assistant in the state(s) of employment.
  • Graduate of an accredited physical therapist assistant program approved by the American Physical Therapy Association.
  • Minimum of six (6) months experience as a Physical Therapist Assistant.
  • Demonstrated written, verbal communication skills, and interpersonal skills.
  • Licensed driver with automobile insured in accordance with state/Agency requirements. Reliable transportation with good driving record.
