Job description

HCBS Community Partnerships & Referral Manager

Northeastern Home Group LLC is seeking an experienced Minnesota HCBS professional to lead referral development, community outreach, case-manager relationships, and referral-partner communication.

We are especially interested in candidates who have previous experience as a Waiver Case Manager, MCO Care Coordinator, Social Worker, MnCHOICES professional, Case Management Supervisor, or HCBS Program Manager and understand how Minnesota Home and Community-Based Services operate.

This is not a traditional marketing position.

We are looking for someone who understands the case-management and care-coordination side of HCBS and can use that experience to build strong professional relationships, generate appropriate referrals, maintain excellent communication with case managers, and help ensure referrals move successfully from initial inquiry to service start.

The ideal candidate will be comfortable working in the community, attending events, meeting with case managers and care coordinators, developing relationships with hospitals and social workers, and representing Northeastern Home Group professionally.

Key Responsibilities

  • Build and maintain relationships with waiver case managers, MCO care coordinators, social workers, county and tribal staff, hospitals, clinics, discharge planners, case-management agencies, and community organizations.
  • Generate appropriate HCBS referral opportunities.
  • Follow up with existing referral partners and develop repeat referral relationships.
  • Attend community meetings, networking events, provider meetings, resource fairs, hospital meetings, and other outreach opportunities.
  • Conduct outreach to hospitals, social workers, discharge planners, counties, MCOs, community organizations, and other potential referral sources.
  • Educate referral partners about Northeastern Home Group's services, availability, and service capabilities.
  • Respond promptly to referral inquiries.
  • Track referrals from initial contact through intake, authorization, staffing, and service start.
  • Maintain regular communication with case managers and care coordinators throughout the referral and service-start process.
  • Help ensure required provider documents, updates, service information, and other appropriate communications are sent to case managers or care coordinators.
  • Track outstanding case-manager requests and follow up until resolved.
  • Identify missing authorizations, support-plan information, signatures, documentation, or communication that may delay a service start.
  • Coordinate with internal leadership, Designated Coordinators, Designated Managers, intake staff, and staffing staff to resolve referral barriers.
  • Monitor referral response time, qualified referrals, referral conversion, new client starts, and repeat referrals.
  • Identify why referrals are delayed, declined, lost, or successfully converted.
  • Gather feedback from case managers, clients, staff, social workers, and community partners.
  • Help identify service gaps and opportunities for improvement.
  • Maintain professional and timely communication with referral partners regarding appropriate client-related matters.
  • Help Northeastern Home Group build a reputation as a responsive, organized, reliable, person-centered HCBS provider.

Preferred Experience

Strong candidates may have experience in:

Previous experience as a Minnesota waiver case manager or MCO care coordinator is strongly preferred.

What We Are Looking For

We want someone who understands both sides of the relationship between a provider and a case manager.

The right person should understand what case managers need from providers, how referrals move through the HCBS system, what can delay services, and why strong provider communication matters.

The individual should also be outgoing and comfortable developing new relationships.

You should be comfortable calling case managers, visiting organizations, introducing yourself to social workers and discharge planners, attending professional events, following up consistently, and building relationships over time.

You must be organized enough to make sure referrals and case-manager communications do not fall through the cracks.

Compliance and Service Quality

Growth must never come at the expense of client choice, safety, staffing capacity, authorization requirements, documentation, service quality, or compliance.

This position is responsible for helping communication and referrals move efficiently, but service acceptance and service-start decisions must remain coordinated with appropriate operational and compliance leadership.

The employee must never promise services or service-start dates unless staffing, authorization, service compatibility, and required internal approvals have been confirmed.
