About the PositionThe Re-Entry Specialist provides support for adults navigating recovery and re-entry after incarceration. Working closely with clinical and peer teams, this role assesses needs, develops transition plans, and coordinates care. The focus is on helping members build independence, achieve stability, and lead purposeful, self-sufficient lives.
