Job description

Essential Duties & Responsibilities:
  • Responsible for initial OT evaluations, reassessments and the development of a treatment plan which is consistent with participant needs.
  • Determines participant’s mental and cognitive status, and takes into account psycho-social issues pertaining to occupational therapy program.
  • Completes Home safety Assessments for initial, annual, and change in status.
  • Identifies DME in the home, and follows policy to log inventory and /or repair/replace damaged equipment.
  • Acts as an advisor to the Recreation staff in planning and implementing the physical activity program for participants.

Qualifications and Requirements:
  • Bachelor degree in occupational therapy required.
  • Must maintain valid license to practice occupational therapy in Massachusetts.
  • Minimum of 2 years geriatric experience in health care or community services setting. Home health and outreach or marketing experience preferred.
  • Must be able to travel to referral sites and make visits to prospective enrollees in a variety of home and institutional settings.

Pay:
  • $38.82 - $60.18/hr based on experience.
