Move Easy Physical Therapy is looking for a dedicated and experienced Senior Home Care Physical Therapist to join our team in Broomfield, Colorado. Work Environment

Your work will primarily be conducted onsite, directly within the homes of patients across Broomfield, Colorado. This unique setting allows you to establish meaningful relationships with clients, assessing their home environment and tailoring treatments accordingly. Key Responsibilities
  • Conduct thorough assessments of patients' physical abilities and limitations in their home environments, developing personalized treatment plans to address individual needs.
  • Utilize expert manual therapy techniques and therapeutic exercises, empowering patients to improve their mobility and regain independence.
  • Educate patients and their families on treatment goals, progress, and strategies for enhancing recovery outside of therapy sessions.
  • Maintain meticulous records of treatment plans, patient progress, and outcomes in compliance with relevant documentation practices, ideally using an EMR system such as WebPT.
  • Manage a patient load of 1-2 clients per hour, allowing ample time for quality interactions and comprehensive care.
  • Engage in ongoing professional development, attending staff meetings and participating in team training sessions to share insights and improve service delivery.
Patient Population / Client Focus

The clients you will work with at Move Easy Physical Therapy include a diverse range of individuals, from post-surgery patients to those with chronic conditions affecting their mobility. Many clients are older adults seeking assistance following injuries or surgeries, as well as those with neurological disorders that require specialized interventions. Your expertise will be pivotal in devising treatment approaches that align with each patient’s unique life circumstances.

Qualifications
  • A Master's or Doctorate Degree in Physical Therapy from an accredited institution.
  • An active Physical Therapist license in good standing in the state of Colorado.
  • A minimum of two years of clinical experience, ideally within a home care setting or with direct patient interactions.
  • Proficiency in manual therapy techniques and the ability to effectively utilize electronic medical record (EMR) systems, with preference given to those familiar with WebPT.
Preferred Experience
  • Demonstrated expertise in geriatric physical therapy and experience with home-based rehabilitation.
  • Strong communication skills related to patient education and family involvement in care plans.
  • Ability to manage time and prioritize patient care effectively, adapting treatment methodologies as required.
Compensation and Benefits
