Key Responsibilities

• Conduct thorough assessments during individual counseling sessions, utilizing video chat and telephone interaction to evaluate clients' mental health needs.
• Develop and implement customized treatment plans tailored to each client’s unique circumstances and goals, employing evidence-based interventions.
• Lead therapeutic sessions focused on grief and loss, providing emotional support and guidance, especially within the hospice context.
• Advocate for clients in various capacities, assisting them in setting realistic and achievable goals for personal growth.
• Coordinate with healthcare professionals, including nurses and doctors within the hospice center, to ensure a holistic approach to each client's care.
• Document progress and treatment effectiveness in compliance with organizational protocols and state regulations.
• Participate in regular case conferences and team meetings to share insights and develop integrated care strategies.

Patient Population / Client Focus

This position primarily serves clients facing significant life challenges, including terminal illness and end-of-life transitions. Qualifications

• Master’s degree in Social Work from an accredited institution.

Compensation and Benefits

This full-time role offers a competitive salary ranging from $100,000 to $105,000 annually, reflecting your experience and expertise.
