Position Overview
Metro Community Health Centers is seeking a Family Nurse Practitioner (FNP) to provide high-quality primary care services to patients of all ages. The FNP will work collaboratively with physicians and a multidisciplinary care team to deliver patient-centered care to the communities we serve.
This is an excellent opportunity for a provider passionate about community health, preventive care, and serving underserved populations.
Responsibilities
  • Provide comprehensive primary care services including assessment, diagnosis, and treatment of acute and chronic conditions.
  • Perform physical exams and obtain detailed patient histories.
  • Order and interpret laboratory tests and diagnostic studies.
  • Prescribe medications and develop treatment plans in accordance with NYS scope of practice.
  • Provide preventive care including health screenings, immunizations, and patient education.
  • Coordinate care with physicians, specialists, behavioral health providers, and care teams.
  • Maintain accurate documentation in the electronic health record (EHR).
  • Participate in quality improvement and population health initiatives.
  • Ensure compliance with all federal, state, and organizational policies.
Qualifications
  • Master's Degree in Nursing (MSN) from an accredited Nurse Practitioner program.
  • Current New York State Nurse Practitioner license.
  • Board Certification as a Family Nurse Practitioner (FNP).
  • DEA license or ability to obtain.
  • Experience in primary care, community health, or FQHC settings preferred.
  • Strong clinical, communication, and patient engagement skills.
  • Experience with electronic health records (eClinicalWorks preferred).
  • Bilingual skills a plus.
