Your Job:
In this highly technical, fast-paced, and challenging position, you'll collaborate with multidisciplinary team members to provide the very best care for our patients. The Community Health Worker provides proactive individualized coaching and resource linking to a group of patients via the telephone and/or home visits with an end goal of affecting patient healthy living and behavioral change.
Your Job Requirements:
High school diploma or equivalent, Bachelor degree preferred
2 years health coaching, wellness, health promotion, health outreach experience. Health education or weight management background preferred
Community Health Worker Certificate or ability to obtain within one year from date of hire
Your Job Responsibilities:
