We are currently seeking a Full Time Clinically Licensed Therapist to join our team in our Behavioral Health Clinic, located in Fort Wayne, Indiana.


About the Role of a Therapist III:

As a Therapist, youll play a vital role for the provision of clinical services to individuals in their respective areas, primarily children, adults and/or families with mental health issues and/or dual diagnosis, or individuals with substance use/dependence issues in both the outpatient and residential setting.


Therapist III Key Responsibilities:

  • Conduct comprehensive clinical assessments, mental status exams, and diagnoses
  • Develop and implement individualized treatment plans in collaboration with patients
  • Provide individual, family, couples, and group counseling services
  • Deliver therapeutic services aligned with treatment plans
  • Collaborate with multidisciplinary teams, courts, schools, and community agencies to coordinate care
  • Communicate effectively with patients, staff, and external partners both verbally and in writing
  • Maintain accurate, timely, and compliant clinical and administrative documentation
  • Adhere to organizational policies, ethical standards, and state and federal regulations

What Were Looking For in a Therapist III:

Were seeking compassionate, motivated individuals who are passionate about whole-person care and thrive in a collaborative, team-based environment. Experience in behavioral health or social services is a plus.

  • Masters Degree in Social Work, Mental Health Counseling, Marital & Family Therapy, or related Behavioral Science field of study as required for licensure
  • Full non-probationary license in one of the following: LCSW, LMHC, LMFT, LCAC
  • Valid drivers license, a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance required.
