Schedule: Friday, Saturday, and Sunday 8:00pm to 8:00 am

WHAT YOU WILL LOVE DOING:
You will provide skilled nursing care in a patient's home as directed by the medical director and/or attending physician. You'll manage and facilitate resource applications to meet patient needs. If there are minimal to no after-hours needs during the shift, you will support chart review and patient phone calls from your home.
Observes and monitors patient conditions and performs comprehensive assessments to develop an individualized care plan and adjusts as needs change.
Evaluates the treatment and medical condition of the patient according to the plan of care.
Determines if the level of care being provided meets the patients' needs and facilitates level of care changes including the management of after-hours Routine, General Inpatient, Respite and Continuous Care.
Administers medication as prescribed by the physician and collaborates on required updates to the plan of care to achieve comfort and symptom control.
Prioritizes visits for high-risk patients.
Manages multi-disciplinary care as applicable while promoting continuity of care with collaborative determinations on level of care provisions, additional resource need identification, and schedule management.
Educates the patient and family on the disease process using teach back methods to ensure patient and family understanding.
Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
Completes chart reviews and answer patient calls, to support patient care and regulatory compliance.
HARD & SOFT SKILLS:
Compassionate communicator with a positive attitude.
Patience is a virtue when working with patients, families, physicians, and coworkers.
Attention to detail is critical, as is being observant and following directions.
REQUIREMENTS:
Registered Nurse with current license in the state of employment.
2 years of hospice experience preferred.
Previous experience with clinical management, hospice case management, or other relevant clinical experience.
Valid driver's license and auto insurance in your name as a driver.
Designated workspace at home to assist with chart reviews if there are minimal to no after-hours needs.
Capable of all physical demands.
