We are seeking a dedicated Home Health RN Case Manager to deliver personalized patient care in a supportive, community-based environment. About the Role: Advance Home Health Nursing and Case Management Care

As a Home Health RN Case Manager, you will coordinate, implement, and evaluate individualized care plans for patients in their homes. Your expertise and empathetic communication will ensure safety, maximize health outcomes, and support both patients and their families throughout the care journey.

Key Responsibilities: Deliver High-Quality Patient Care at Home
  • Conduct comprehensive patient assessments and develop evidence-based care plans tailored to each individual’s needs.
  • Manage a caseload of patients, ensuring consistent, high-quality nursing care, and timely completion of required home visits.
  • Monitor patient progress, revise care plans as needed, and coordinate with healthcare providers and caregivers to achieve optimal outcomes.
  • Educate patients and their support systems on disease management, medications, nutrition, and preventive care in the home environment.
  • Maintain accurate and timely documentation in compliance with regulatory and organizational standards.
  • Provide compassionate support and advocacy for patients and their families, addressing concerns and fostering independence whenever possible.
  • Ensure best practices in infection control, safety, and care transition planning.
Qualifications: Home Health RN Case Management Expertise
  • 1+ years of experience in Clinical Quality and Nursing Experience
  • 1+ years of experience in EMR, Other EMR and TruChart EHR
  • Valid RN from the state of PA
  • Valid DL
  • All of the following: AHA BLS and AHA ACLS
