Key Responsibilities

  • Develop, implement, and monitor integrated treatment plans that address both psychiatric and medical needs.
  • Facilitate individual and group therapy sessions using evidence-based practices suited to patients’ unique circumstances.
  • Collaborate closely with medical providers to ensure coordinated care and optimize treatment interventions.
  • Provide immediate crisis intervention and tailored support during mental health emergencies.
  • Educate patients on the interaction between mental health and physical conditions, empowering them with tools to manage their overall health.
  • Maintain meticulous and confidential documentation in compliance with relevant legal and ethical standards.
  • Actively engage in team meetings and interdisciplinary care planning to refine treatment approaches and improve patient outcomes.
  • Stay informed about best practices, recent research findings, and developments in integrated care.
Patient Population / Client Focus

This role is specifically focused on treating individuals facing a variety of challenges stemming from co-occurring medical and psychiatric disorders. As a member of our team, you will work with a diverse patient population that includes those coping with chronic pain, severe mental illnesses, and substance use disorders. Your expertise and empathetic approach will be critical in helping these individuals achieve improved mental health and overall wellness.

Qualifications
  • A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
  • Current and valid licensure as an LMFT, LCPC, or LCSW in the state of Illinois.
  • Strong proficiency and clinical experience in addressing mental health concerns in individuals with complex medical conditions.
  • Demonstrated ability to collaborate effectively with healthcare professionals in an integrated care setting.
  • Excellent communication and interpersonal skills, fostering trust and effective relationships with patients and team members.
  • Commitment to maintaining professional boundaries, adhering to ethical guidelines, and ensuring patient confidentiality.
  • Familiarity with electronic health record (EHR) systems and telehealth tools.
  • Ability to work collaboratively as part of an interdisciplinary team in a fast-paced environment.
Preferred Experience
  • At least one year of post-graduate experience providing mental health services in a clinical setting.
  • Experience in integrated care models and working in multidisciplinary teams.
  • A proven track record of successful engagement with patients exhibiting complex health needs.
Compensation and Benefits

The position offers a competitive salary range of USD 75,000 to 100,000 annually, commensurate with experience and qualifications.
