Position Overview

Lifeline Community Care is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our team in Jacksonville, Florida. Work Environment

This role will primarily be remote, offering the flexibility to provide care from various settings while collaborating with a multidisciplinary team from the safety of your home. You will interact with clients via telehealth platforms, supporting their needs effectively through modern communication tools.

Key Responsibilities
  • Provide individual, group, and family therapy sessions virtually and in community settings, addressing the needs of clients with significant mental health challenges.
  • Conduct thorough, evidence-based assessments and screenings, ensuring the appropriate linkage to comprehensive integrated care plans tailored to each client's unique background and circumstances.
  • Maintain an active caseload, monitoring client progress and adapting treatment strategies to meet changing needs, while documenting encounters accurately and on time.
  • Incorporate crisis intervention strategies as needed, poised to respond to emergencies with sensitivity and professionalism.
  • Collaborate with a team of professionals to enhance patient care and improve service delivery through active participation in team meetings and quality improvement committees.
  • Ensure compliance with ethical standards and organizational regulations, maintaining the highest level of professional integrity in all client interactions.
  • Utilize organizational resources efficiently to facilitate client support and care coordination, engaging with families and community resources when necessary.
Patient Population / Client Focus

This role focuses on working with under-served populations experiencing a spectrum of mental health issues, including but not limited to anxiety disorders, depression, PTSD, and other serious emotional disturbances. Our clients come from diverse backgrounds, requiring a culturally competent approach that respects their traditions and promotes their empowerment in the recovery process.

Qualifications
  • Must possess a valid LCSW license and have at least two years of clinical experience in a similar setting.
  • A Bachelor’s degree in social work or a related field is required; advanced degrees are a plus.
  • Exceptional communication skills, both verbal and written, with the ability to engage effectively with clients from diverse backgrounds.
  • Proficiency in technology and telehealth systems that facilitate remote service delivery.
  • Demonstrated organizational and planning skills, with a focus on goal setting and outcome tracking.
Preferred Experience
  • Experience in substance abuse treatment or crisis intervention is highly desirable.
  • Knowledge of community resources and the local healthcare landscape in Jacksonville will be advantageous.
  • Familiarity with electronic health records (EHR) systems for maintaining accurate and up-to-date client information.
Compensation and Benefits

The salary for this full-time position ranges from USD 70,000 to 85,000 annually, commensurate with experience and qualifications.
