Position Summary:

The Social Worker is responsible for specific duties as assigned to him by the Coordinator of Social Services.  This includes, but is not limited to, assessing and meeting the psychosocial needs of the residents and family intervention as needed, assisting with Medicare and Medicaid procedures and working as part of an interdisciplinary team, as well as, other requirements as stated in IV below.

Key Responsibilities:

• The Social Worker is supervised by the Coordinator of Social Services.  The job duties and responsibilities are to be performed while observing the following standards: job standard (knowledge, ability, skills), technical skill, quality, quantity, judgement/problem solving, decision making, and initiative.  The responsibilities include, but are not limited to the following:
• Implements a plan of care that meets the emotional social and psychological needs of each resident assigned.
• Prepares a social history as required on each admission and provides periodic social progress notes on each resident assigned.
• Provides counseling service for the resident and/or family members maintaining confidentiality of information.
• Acts as an advocate for the resident and their family.
• Serves as a liaison between the facility and the families of prospective residents.
• Arranges for admission of new residents in conjunction with administration and the Admission Assistant.
• Interviews, screens and provides necessary background information on perspective residents and evaluates resident's appropriateness for placement.
• Acts as a liaison between financial assistance agencies, insurance carriers, residents and families.  Assists in determining resident eligibility for financial aid and assists the resident and family with completion of placement necessary to secure financial assistance.
• Coordinates Medicare notification procedures.
• Provides an orientation to the facility for new residents.  This includes services, information procedures, Bill of Rights, interpreting Resident Rights to resident and family, introductions to staff and other residents.
• Obtains medical approval for Medicaid residents who need transportation to and from medical appointments.
• Works with Finance Department in providing and maintaining resident personal funds.
• Completes necessary paperwork, including transfer summary for hospitalized residents.
• Disseminates educational information to resident and family as indicated.
• Assists in developing programs that will benefit residents' emotional, social, psychological, mental, and physical well-being.
• Attends weekly resident care planning meetings, makes appointments for resident/family to attend.
• Keeps departmental records, including social histories, progress notes, psychosocial assessments, statistics and summaries as necessary.
• Assists resident/family in discharge planning.
• Assists with personal affairs and provides personal services for those residents who have no family or for those with family living out of town, including burial arrangements when necessary.
• Facilitates needed communication between residents, families and staff.
• Coordinates and monitors needed services for individual residents to assure optimism level of emotional physical and psychological well being and independence.
• Involves resident, appropriate disciplines and administration regarding matter such as bed retention, room changes, transfers and/or discharges.
• Interprets residents needs, behaviors and rights by extending professional intervention to all levels of staff by suggesting positive approaches, including alternatives to use of physical and chemical restraints.
• Coordinates and facilitates the referrals of residents for needed/requested services and resources not available in the facility.
• Organizes and facilitates bereavement counseling for residents, families and other effected individuals.

Qualifications: 

• Education and Training - A minimum of a baccalaureate degree, from a recognized college or university, in social work or related field.  A Masters candidate in social work may be considered.  This person must be willing to keep informed of legislative changes and programs affecting the fields of Social Services, Gerontology, and Long Term Care, and attend appropriate programs. 
• Experience -Experience in geriatrics is preferred in institutional and/or home care setting.
• Character - The individual must possess good moral character, pleasing personality, good judgement, dependability, professional business ability, maturity, retain appropriate information in strictest confidence, and a neat appearance.
