Join Kettering Home Care as a Physical Therapist and provide one-on-one care that helps patients recover safely and confidently in the comfort of their own homes.
As a Home Health PT, you'll have the opportunity to build meaningful relationships with patients and families, create individualized care plans, and see the direct impact of your work throughout each patient's recovery journey.

What You'll Do
Provide skilled physical therapy services in patients' homes
Complete evaluations and develop individualized plans of care
Help patients improve mobility, strength, safety, and independence
Educate patients and families on therapy goals, equipment use, and injury/disease management
Monitor patient progress and adjust treatment plans as needed
Collaborate with physicians, PTAs, nurses, and the interdisciplinary care team
Complete documentation in accordance with agency and Medicare guidelines
Support continuity of care through admissions, transfers, and discharges
Qualifications
Current Physical Therapist license in the state of employment
Minimum of 1 year of PT experience in an acute care setting required
Home health experience preferred
Valid driver's license and auto insurance
Strong communication, time management, and clinical judgment skills
Comfortable working independently while staying connected to a supportive team
