Roadrunner Hospice is seeking a compassionate, organized, and empathetic Bereavement Coordinator to provide support and guidance to families and loved ones following the loss of a patient. This vital role is responsible for developing and coordinating bereavement services that promote healing, emotional well-being, and ongoing support for those navigating grief.

At our agency, we believe that hospice care extends beyond the patient and includes caring for families through every stage of the journey.

Position Summary

The Bereavement Coordinator oversees and implements the agency's bereavement program, ensuring that families receive individualized support consistent with hospice standards and regulatory requirements. The ideal candidate is a skilled communicator with experience in grief counseling, social services, chaplaincy, mental health, or hospice care.

Essential Responsibilities

  • Coordinate and manage the agency's bereavement program.
  • Conduct bereavement risk assessments for patients' families and caregivers.
  • Develop individualized bereavement plans and support services.
  • Provide grief support through phone calls, visits, support groups, and written communication.
  • Facilitate bereavement support groups and educational programs.
  • Maintain accurate and timely documentation in accordance with agency policies and regulatory requirements.
  • Collaborate with the interdisciplinary team to ensure continuity of care.
  • Recruit, train, and support bereavement volunteers as needed.
  • Assist with community outreach and education regarding grief and bereavement services.
  • Monitor program effectiveness and identify opportunities for improvement.
