The employer is a growing pediatric healthcare organization in San Diego County seeking dedicated, licensed Occupational Therapists (OT / OTR / OTR-L) to join a community-based clinical team. In this home health OT role, you will deliver early intervention and pediatric occupational therapy services to children ages 2 through 10 — in their natural environments, including the family home and community settings where real functional growth happens. Out-of-state OTs who are open to working in the San Diego area are welcome. California OT licensure costs are reimbursed in full for applicants pursuing licensure by endorsement, and relocation assistance of up to $2,500 for candidates hired from outside San Diego County is available.

This full-time W-2 position starts in-person and transitions to a hybrid occupational therapist schedule — approximately 50% in-person field visits and 50% remote telehealth OT — after roughly 3 months, once rapport is established with clients and families. For the right candidate, this hybrid model may mean that a permanent, full-time relocation to San Diego is not required in the long run. With adequate planning and scheduling, clinicians who demonstrate strong performance and organizational discipline during the introductory phase may be able to fulfill their in-person obligations while maintaining flexibility in where they are based — growing with the organization on their own terms.

RESPONSIBILITIES

  • Provide pediatric occupational therapy services to children ages 2–10 with developmental delays, autism spectrum disorder (ASD), sensory processing disorders, and related diagnoses.
  • Evaluate each child's functional status including fine motor skills, gross motor skills, sensory processing, visual-motor coordination, self-care, and activities of daily living (ADLs).
  • Assess the home and natural environment for barriers to independence and recommend adaptive equipment as needed.
  • Develop individualized, evidence-based treatment plans and establish measurable functional goals in collaboration with the supervising OTR and interdisciplinary team.
  • Deliver direct in-home OT therapy and community-based occupational therapy during the in-person phase of the schedule.
  • Transition to telehealth occupational therapy sessions as part of the hybrid OT schedule once established.
  • Collaborate with families, caregivers, educators, SLPs, BCBAs, and interdisciplinary team members to support skill generalization across environments.
  • Teach and re-train children in daily living skills using naturalistic, functional approaches within their primary environment.
  • Supervise Certified Occupational Therapy Assistants (COTAs) where applicable and as California licensure permits.
  • Maintain accurate, timely clinical documentation and progress notes per AOTA and CBOT standards.

WORK SCHEDULE

  • First ~3 Months (Onboarding): Primarily in-person — building rapport with pediatric clients, families, and team members across the San Diego County service area.
  • After ~3 Months (Hybrid Phase may begin): OTs may transitions to a 50/50 hybrid schedule — approximately half in-person home health visits, half remote telehealth OT, documentation, and care planning.

EXPERIENCE

  • Pediatric occupational therapy or early intervention experience: 1 year (Preferred)
  • Pediatric, home health or community-based occupational therapy experience (Preferred)
  • Experience with autism spectrum disorder (ASD), sensory processing, or developmental delays (Preferred)

LICENSE / CERTIFICATION

  • Active California OT license (OTR / OTR-L) or Out-of-state equivalent — Required for application to be reviewed.
  • NBCOT certification (OTR) — Required
  • AOTA membership or familiarity with AOTA practice standards (Preferred)
  • California Board of Occupational Therapy (CBOT) compliance required
  • Driver's License — Required
  • Proof of active professional liability (malpractice) insurance required prior to start date — Required
  • Out-of-state OT license — California OT licensure by endorsement fully reimbursed. Licensed OTs in Alabama, Arizona, Texas, Florida, Georgia, Mississippi, Ohio, Arkansas, Kansas, Colorado, Nevada, Oregon, Washington, and all other states are encouraged to apply.
  • Supervision eligibility per CBOT and AOTA standards (Preferred)

Reimbursed at the applicable IRS standard mileage rate and rules.
  • Meaningful work with children ages 2–10 in natural environments — early intervention, home health OT, and community-based occupational therapy.
