Job description

The Holocaust Survivor Homecare Case Worker plays a vital role in providing compassionate, comprehensive support to Holocaust Survivors enrolled in JCCGCI’s HSSS Programs. This position requires empathy, strong organizational skills, and a commitment to improving the quality of life for Holocaust Survivors.

 

Job Responsibilities:

  • Maintain a Homecare Case Work caseload.
  • Conduct comprehensive home visits to intake and evaluate clients’ physical, emotional, and social needs.
  • Identifying unmet needs of each Homecare client (in addition to Homecare, such as health, social, mental health, financial, quality of life and other needs).
  • Develop individualized care plans tailored to each survivor’s circumstances and preferences.
  • Maintain consistent communication with clients and their families through regular check-in calls and in-person visits.
  • Collaborate with healthcare providers, community agencies, and other support services to ensure clients receive quality care and appropriate resources.
  • Assist in arranging homecare services, counseling, financial assistance, and other vital support systems.
  • Advocate on behalf of clients to ensure they receive respectful, comprehensive, and compassionate care.
  • Maintain accurate, timely, and complete client service records in accordance with funder, regulatory, and agency requirements.
  • Prepare regular reports summarizing client status, service delivery, and any ongoing needs or concerns.
  • Participate in staff meetings and trainings as required.
  • Additional duties, as assigned.

 

Job Requirements:

  • High school education required; college degree in social work (BSW) or a related field or equivalent experience in social services or case management preferred.
  • Experience working with older adults, trauma survivors, or vulnerable populations is highly desirable.
  • Strong written and verbal interpersonal and communication skills, with empathy and cultural sensitivity.
  • Ability to manage multiple cases, and maintain organized, timely documentation.
  • Ability to prioritize tasks and manage time effectively.
  • Capacity to work both independently and collaboratively in a fast-paced and dynamic environment.
  • Knowledge of local social services, healthcare systems, and community resources.
  • Proficiency in Microsoft Word, Excel, and data entry systems.
  • Bilingual Yiddish and/or Russian preferred.

Pay: $27.00 - $28.00 per hour.

Schedule: 35 hours per week.

This role entails In-Office work with travel to clients' homes required.
