Responsibilities:
* EMR Review and processing of clinical documentation for compliance
* Education/Training of clinicians
* Coordination of Care; MDs/SWs/DC Planners/Clinicians
* Reviewing referrals for clinical acceptance
* QAPI performance Improvement
* Care Team Conference
* Transmitting OASIS
* Oversight of clinical and services
* ACHC Accredited, IL Licensed Agency for Home Health and Home Care
* Clinical Coverage
* Clinical Competencies/Reviews
Qualifications:
* Registered Nurse (RN) with current state license
* 2-3 years of home health experience required
* Strong leadership, communication, and problem-solving skills
* CPR Certification, valid driver's license and auto insurance
