Key Responsibilities

• Design and implement personalized support packages that aid clients in overcoming challenges and dependencies, carefully considering their unique circumstances.
• Conduct comprehensive assessments through interviews with service users to evaluate their current emotional, psychological, and social conditions.
• Manage each case individually by setting specific, measurable goals in collaboration with clients, ensuring tailored interventions are deployed effectively.
• Monitor client progress, regularly revising treatment plans based on their evolving needs and outcomes, integrating feedback during counseling sessions.
• Provide insightful information and guidance on appropriate actions during therapy sessions, while ensuring a supportive therapeutic atmosphere.
• Maintain meticulous records and documentation, offering detailed reports on client progress to uphold accountability and facilitate care continuity.
• Participate actively in case conferences and, when necessary, supply evidence in court as a key expert witness.
• Act as a primary coordinator for clients, collaborating with other specialists and community services to optimize support offerings and ensure a holistic care approach.

Patient Population / Client Focus

This role is primarily dedicated to working with individuals facing serious health challenges in the hospice setting. You will assist patients and families navigating end-of-life decisions, grief, and emotional distress. Your work will directly impact the quality of life and provide emotional support to both clients and their loved ones during these pivotal times, promoting dignity, hope, and healing.

Qualifications

• A Bachelor’s Degree in Social Work, Psychology, Sociology, or a closely related field. A Master’s Degree in Social Work (MSW) is preferred.
• Demonstrated experience as a Social Worker, ideally within mental health and hospice care settings.
• A solid understanding of social theories and best practices surrounding mental health interventions.
• Exceptional social perceptiveness and a high degree of empathy to effectively relate to diverse populations.
• Strong interpersonal skills to build trusting relationships with clients and collaborate with multidisciplinary teams.
• Resilience and an analytical mindset to assess dynamic situations and respond appropriately.
• Willingness to complete background checks as part of the hiring process.

Preferred Experience

• Experience specifically in hospice or palliative care settings will be an advantage.
• Familiarity with electronic health records (EHR) and case management software systems.
• Previous experience in collaborating with community resources and services.

Compensation and Benefits
