Key Responsibilities

• Utilize your expertise to conduct thorough assessments and formulate independent psychosocial diagnoses, focusing on each patient’s specific needs.
• Collaborate with patients to develop individualized, patient-centered treatment plans that empower them on their journey toward recovery.
• Implement evidence-based interventions tailored to patients’ unique circumstances, utilizing techniques such as motivational interviewing and problem-solving treatment.
• Work synergistically with other healthcare providers, including psychologists and primary care physicians, to ensure a holistic approach to patient care.
• Monitor and document patient progress through regular assessments, both virtually and in-person, to track treatment goals effectively.
• Support medication management services by collaborating with prescribing medical professionals, emphasizing adherence and addressing potential side effects.

Qualifications

• A Master's degree in Social Work from an accredited institution.
