Position Overview

Infinite Ties Wellness is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in providing critical support within our Hospice Center. This full-time position is remote, offering the flexibility to work efficiently while enhancing the quality of life for patients and families navigating end-of-life challenges. The salary for this role ranges from USD 75,000 to 100,000 annually, commensurate with experience and qualifications.

Key Responsibilities

• Conduct in-depth assessments to determine the social, emotional, and psychological needs of clients and their families facing terminal illnesses.
• Provide individualized and group counseling utilizing evidence-based therapeutic approaches, tailored to the specific challenges of clients in hospice care.
• Establish and implement comprehensive service plans that align with clients’ goals, taking into account their unique circumstances.
• Actively advocate for clients’ rights and facilitate access to necessary resources, including counseling, financial assistance, and community services.
• Contribute to the organization’s program development, evaluation, and grant reporting to ensure our services meet the evolving needs of our clients.
• Collaborate with healthcare professionals, community organizations, and volunteers to enhance the array of services available to clients.
• Participate in ongoing training, team meetings, and community outreach activities to advance our collective knowledge and efficacy.

Patient Population / Client Focus

As an LCSW at Infinite Ties Wellness, you will specifically focus on clients receiving hospice care. Your work will involve supporting patients and their families through some of the most challenging times in their lives. You will provide them with compassionate counseling, guidance, and resources aimed at fostering dignity, comfort, and emotional well-being during the process of hospice and palliative care.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Demonstrated understanding of social justice principles and trauma-informed care.
• Exceptional communication skills with a strong emphasis on empathy and active listening.
• Organizational skills that enable effective multitasking and prioritization in a dynamic environment.

Preferred Experience

• Previous experience in a nonprofit organization, particularly in hospice or palliative care settings, is highly desirable.
• Familiarity with community resources and social service programs relevant to hospice care.
• Experience in advocacy and policy development related to social justice and healthcare.

Compensation and Benefits

Infinite Ties Wellness offers a competitive salary in the range of USD 75,000 to 100,000 per year, complemented by a sign-on bonus.
