Job description

About the Role:
We are seeking a highly skilled Physical Therapist to join our team as a PT for Home Health. In this role, you will be responsible for providing exceptional care to patients in their homes, helping them to regain their strength and mobility. You will work closely with physicians and other healthcare professionals to develop and implement individualized treatment plans that meet the unique needs of each patient. Your ultimate goal will be to help patients achieve their maximum potential and improve their quality of life.
Minimum Qualifications:
  • Current state license as a Physical Therapist
  • At least 1 year of experience in home health or a related field
  • Strong knowledge of anatomy, physiology, and kinesiology
  • Excellent communication and interpersonal skills
  • Ability to work independently and manage time effectively
Responsibilities:
  • Conduct thorough evaluations of patients' physical conditions and develop individualized treatment plans
  • Provide hands-on therapy to patients in their homes, including exercises, stretches, and other therapeutic techniques
  • Monitor patients' progress and adjust treatment plans as needed
  • Educate patients and their families on proper techniques for performing exercises and other therapeutic activities
  • Collaborate with physicians and other healthcare professionals to ensure the best possible outcomes for patients

Skills:
As a Physical Therapist for Home Health, you will utilize your strong knowledge of anatomy, physiology, and kinesiology to develop and implement individualized treatment plans for patients in their homes. You will use your excellent communication and interpersonal skills to educate patients and their families on proper techniques for performing exercises and other therapeutic activities. Your ability to work independently and manage time effectively will be essential in ensuring that patients receive the care they need in a timely and efficient manner. Additionally, any specialized certifications or experience working with geriatric patients will be highly beneficial in this role.
