Job description

Job Mission:

To provide oversight and supervision of the daily operations and staff activities for Community Health programs to address social determinants of health. Further, Supervisor is responsible for using independent judgment and discretion regarding marketing of the programs, monitoring of the problems, dispute resolution of programs, and team supervision.

Performance Standards:

1. Supervise assigned Health Integrated Services for Food Bank, Medical Nutrition Therapy, Medical Transportation, and Psychosocial Support operations and functions to ensure the programs are providing compliant and quality services in accordance with OLHSA’s and funding sources’ service standards, policies, and procedures.

2. Responsible for database administration, reporting, and internal control activities.

3. Supervise three or more staff to ensure that responsibilities are carried out in a cohesive, cooperative manner to meet the needs of clients. Supervisor shall have hiring, firing and disciplining responsibilities in concert with the Assistant Director -HHN.

4. Ensure that external partner relationships and systems of coordinated care operate successfully, including prominent engagement and leadership in the healthcare continuum.

5. Deliver direct community health services to participants, as needed.

6. Carry out all other duties as directed by supervisor.

Qualifications:

1.  Bachelor’s Degree in human services, health, or related field required, or, minimum of five (5) years of experience providing direct services in HIV care, public health, or related social service fields.

2.   Two (2) years’ experience in management (including recruitment, selection, supervision, retention, and training) required. Three (3) or more years of experience in management preferred.

3. Minimum of two (2) years’ experience working with low income, diverse populations required.

4. Valid Driver’s License, valid automobile insurance and reliable transportation to be use for job related travel are required.

Physical Job Description:

Job requirement in an 8 hour workday

Sit: 6 hours- this position varies daily w/ writing reports/grants, going to meetings

Stand: 1 hour- picking up of supplies

Walk: 1 hour- interaction w/staff, walking to meeting on multiple floors, areas

Movements: YES- Reach above- reaching for binders, books, supplies, files; Crouch- low files; Squat- filing and/or retrieving file folders; Climb- stairs; Shoulder Level- reaching for binders, books, supplies, files; Kneel- for materials; Pushing-pushing cart w/supplies

Weight Carried: up to 10 lbs.- files or materials, lifting nutritional supplements; Weight Lifted: up to 10 lbs. - files or materials, lifting nutritional supplements

Hours & Wage:

$1071.60 per week; 40 hours per week; 52 weeks per year
