About the Role

Reporting to the Clinic Manager, the Nurse Practitioner (NP) provides comprehensive primary health care services to individuals and families accessing care through IIPCT. The NP works independently and collaboratively within an interdisciplinary team to assess, diagnose, treat, educate, and advocate for patients across the lifespan.

This role blends evidence-based clinical practice with culturally responsive care, helping individuals achieve their health goals while respecting Indigenous knowledge, traditions, and community values.

What You'll Do

• Provide comprehensive primary care services, including health assessments, diagnosis, treatment, prescribing medications, and ordering and interpreting diagnostic tests.
• Develop individualized care plans that promote health, prevent illness, and support chronic disease management.
• Deliver culturally safe, trauma-informed, and patient-centred care to individuals and families.
• Provide health education, counselling, and disease prevention strategies.
• Collaborate with physicians, nurses, social workers, mental health professionals, and community partners to coordinate care.
• Participate in case conferences and interdisciplinary team meetings.
• Maintain accurate clinical documentation and electronic medical records.
• Support quality improvement initiatives and evidence-informed practice.
• Participate in community outreach, health promotion activities, and organizational initiatives as required.

What You'll Bring

• Current registration in the Extended Class with the College of Nurses of Ontario (CNO).
• Completion of a recognized Primary Health Care Nurse Practitioner program.
• Current Registered Nurse (Extended Class) designation (RN(EC)).
• Experience providing primary care within a community or primary care setting is preferred.
• Knowledge of chronic disease management, health promotion, and preventive care.
• Experience working with First Nations, Inuit, and Mtis communities is an asset.
• Understanding of Indigenous cultures, histories, and the social determinants of health.
• Commitment to providing culturally safe, trauma-informed, and equitable care.
• Excellent clinical assessment, diagnostic, communication, and critical thinking skills.
• Experience using electronic medical records and other healthcare technology.
• Valid Ontario driver's licence and access to a reliable vehicle.
• Willingness to travel between IIPCT sites and throughout the service area as required.
• Current immunizations and a satisfactory police record check.
• Willingness to participate in Indigenous Cultural Safety training and cultural teachings.
