Key Responsibilities
  • Act as a primary liaison between clients, their families, and outpatient providers to ensure seamless coordination of care.
  • Engage with insurance companies for pre-certification and utilization management, advocating for necessary services on behalf of clients.
  • Contribute to discharge and aftercare planning, ensuring that clients have comprehensive support systems in place upon their transition from care.
  • Provide therapeutic meal support to clients, modeling and fostering a healthy relationship with food and nutrition.
  • Deliver empathetic and affirming care tailored to the individual circumstances of each client, recognizing the impact of their intersecting identities on their health and well-being.
Patient Population / Client Focus

Our patient population primarily includes older adults facing mental health challenges, including but not limited to depression, anxiety, and grief. You will work with clients experiencing unique circumstances that require sensitivity and an understanding of their individual histories and identities, ensuring that they receive respectful and effective therapeutic interventions.

Qualifications
  • A Master's Degree in Social Work from an accredited institution is required.
  • A valid LCSW license in the state of California is mandatory.
  • Proven experience in providing care for older adults, with a strong understanding of the specific challenges they face.
  • Exceptional clinical and diagnostic skills to assess and create effective treatment plans.
  • Strong communication and interpersonal abilities to build rapport with clients, families, and multidisciplinary team members.
  • Capacity to work both independently and collaboratively, adapting to varying needs and circumstances.
Preferred Experience
  • Experience in a hospice setting or with end-of-life care services is a plus.
  • Familiarity with electronic health record (EHR) systems to document patient interactions and maintain treatment plans effectively.
  • Previous engagement with community resources and support services to enhance the care continuum for clients.
Compensation and Benefits
