Position Summary

Hope Hospice is seeking a dynamic and relationship-driven Registered Nurse to serve as a Hospice Clinical Liaison. This role is responsible for building strong partnerships within contracted healthcare facilities, identifying potential hospice referrals, and facilitating timely admissions into hospice care.

The Clinical Liaison provides education and training to healthcare facility staff on hospice services, pain and symptom management, and end-of-life care. This position also supports patients and families by explaining hospice benefits and coordinating admissions in collaboration with the admissions team.

Key Responsibilities

  • Build and maintain relationships with contracted nursing facilities and referral sources
  • Identify and coordinate hospice referrals and facilitate smooth admissions
  • Provide education and in-services on hospice care, pain management, and end-of-life issues
  • Serve as a resource to facility staff, physicians, and community partners
  • Support care coordination for patients transitioning between care settings
  • Ensure accurate and collaborative documentation in facility medical records
  • Participate in community outreach and professional engagement activities - including creating and presenting community presentations

Qualifications

  • Graduate of an approved school of nursing (BSN preferred)
  • Current Registered Nurse (RN) license in the State of Texas
  • Knowledge of hospice and nursing facility regulations
  • Excellent communication, presentation, and interpersonal skills
  • Self-motivated with the ability to work independently
  • Valid Texas driverโ€™s license, reliable transportation, and auto insurance

Working Conditions

This position involves frequent travel to contracted facilities, flexible hours, and participation in an on-call rotation (1-3 times per quarter).
