Our commitment to excellence shapes our vision for future healthcare solutions. We specialize in patient and family-centered care, offering comprehensive services including home healthcare, home hospice, private duty home care, and independent living support. • Designing and implementing individualized physical therapy treatment plans focused on preventing disability, relieving pain, and enhancing muscle function.
  • Supervising Physical Therapy Assistants to ensure effective execution of treatment plans and facilitating interventions that promote patient progress.
  • Evaluating patients’ functionality through rigorous diagnostic tests that assess muscle, nerve, joint, and overall physical capabilities.
  • Conducting detailed assessments of the home environment to make informed recommendations that enhance patient autonomy.
  • Administering various therapeutic interventions, including manual exercises, traction, and therapeutic applications.
  • Fitting patients with prosthetic and orthotic devices as appropriate, providing adjustments and recommendations to enhance their effectiveness.
  • Documenting clinical records, including initial evaluations, progress notes, and discharge summaries, ensuring compliance with state and federal regulations.
  • Engaging in interdisciplinary team meetings to collaboratively plan and evaluate treatment strategies tailored to each patient.
  • Educating patients and their families about home care techniques, the use of therapy devices, and the complete physical therapy program.
  • Contributing to the continuous improvement of services by participating in management meetings and suggesting enhancements to the care delivery process.
Patient Population / Client Focus

The primary clientele at Symbria Rehabilitation Center includes individuals recovering from various health conditions, including post-surgical rehabilitation, chronic pain management, and geriatric care. Our approach emphasizes empathy and understanding, tailoring each therapy encounter to meet the unique needs of every client and their families, ensuring empowerment through education and management of their health journey.

  • Current CPR certification and a valid driver’s license with automobile insurance.
