Acting as a vital liaison between patients and healthcare providers, the CWC plays a crucial role in ensuring access to quality care, personalized patient education, and coordinated services, with a focus on individual health and wellness.

* High School Diploma or GED
Experience:

*Five (5) years of clinical experienc
*Experience in wellness coaching and care coordination two (2) years of experience.
Credentials:

*Graduate of an accredited Medical Assistant/ EMT program
*BLS
AHCA Level II Required
