The Health and Peace Center is seeking a dedicated and compassionate Clinical Social Worker (LCSW) to join our team on a full-time, remote basis. In this crucial role, you will work closely with individuals experiencing significant mental health challenges, providing them with the support and guidance they need to navigate their emotional and psychological struggles. This position not only involves direct clinical work but also emphasizes collaboration with other healthcare providers to ensure comprehensive care for our clients.

About the Organization

Health and Peace Center is a pioneering community health clinic located in Hartford, Connecticut. Our mission is to promote healthier communities by delivering high-quality, patient-centered healthcare services, particularly in medically underserved areas. We firmly believe in providing care regardless of a patient’s ability to pay, making a significant impact on the lives of individuals and families in our community.

Work Environment

This role is designed for those who thrive in innovative and supportive environments. The position allows for flexibility and autonomy as it is primarily remote, and you'll collaborate with a diverse team committed to uplifting our patients. Your role will contribute directly to a workplace culture that values teamwork, open communication, and a shared vision of excellence in patient care.

Key Responsibilities
  • Provide direct clinical services to clients both in-office and within the community who are experiencing symptoms of major mental illness and severe emotional disorders.
  • Engage collaboratively with fellow team members to foster a cooperative environment aimed at achieving shared goals and enhancing patient outcomes.
  • Demonstrate respect and understanding towards clients’ diverse backgrounds, belief systems, and interpersonal styles, ensuring culturally competent care.
  • Navigate the organizational and financial frameworks of Health and Peace Center while interacting with additional external systems, maintaining a focus on delivering effective services.
  • Uphold the highest standards of professional ethics and the code of conduct established by Health and Peace Center.
  • Perform comprehensive, evidence-based screenings and facilitate in-depth assessments when needed.
  • Develop, implement, and monitor integrated care plans, ensuring clients have access to necessary services and that communication among clients, families, and providers is clear and effective.
  • Provide ongoing treatment interventions, including individual, group, and family therapy, tailored to address the clients' specific needs and challenges.
  • Carry a manageable caseload, overseeing client assessments, treatment progress, and ensuring timely interventions as required.
  • Complete necessary documentation, reports, and data submissions promptly to meet organizational standards and improve service delivery.
  • Participate in on-call duties during and outside regular program hours as needed.
  • Contribute to agency-wide committees aimed at enhancing service delivery and implementing quality improvement initiatives.
Patient Population / Client Focus

You will primarily work with a diverse client population, focusing on those experiencing major mental health challenges and severe emotional disorders. This role requires you to apply your expertise in understanding and navigating the complexities of mental health treatment, ensuring that each individual receives a personalized approach to recovery that emphasizes empowerment and informed choice.

Qualifications
  • A Master’s degree in Social Work (MSW) or a related field is required.
  • Active LCSW licensure in the state of Connecticut is mandatory to practice.
  • Proficiency in assessment, diagnosis, and treatment planning for mental health issues, as well as expertise in crisis management.
  • Exceptional verbal and written communication abilities are essential to effectively interact with clients, families, and team members.
  • Strong organizational and planning skills, demonstrating an ability to manage multiple tasks and responsibilities efficiently.
  • Demonstrated cultural competence in working with diverse populations and understanding their unique developmental needs.
Preferred Experience
  • A minimum of 2 years of clinical experience is preferred, with a background in community health settings being advantageous.
  • Bilingual proficiency in English and Spanish is highly desirable to better serve our diverse client base.
Compensation and Benefits
  • Annual base salary ranging from $80,000 to $100,000, commensurate with experience and qualifications.
  • Sign-on bonuses available for qualified candidates.
  • Comprehensive health, dental, and vision insurance plans to support your health and well-being.
  • 401(k) retirement savings plan with company matching contributions.
  • Generous paid time off (PTO) and sick time policies to ensure work-life balance.
  • Life insurance options and profit-sharing opportunities that enhance your financial security.
  • Access to employee discounts with various vendors, enhancing your overall employment experience.
  • Tuition reimbursement for continued education and professional development.
  • Flexible work schedules promoting a healthy balance between work and personal life.
  • Referral bonuses for referring qualified candidates to our team.
