Job description

ESSENTIAL 

  • Provides personal aspects of care to patients in order to increase level of comfort and maintain personal hygiene.  

  • Provides temporary respite and relief to patient’s family from total personal care of the patient. 

  • Assists in maintaining a safe, healthy environment for patients and families.  

  • Meets with Interdisciplinary Team, when appropriate, to assists with review and amendment of the patient’s care plan.  

  • Practices safely and competently within job description.  

  • Completes duties according to the Hospice Aide assignment sheet.  

  • Reports significant changes in patient’s condition to registered nurse (done so for follow- through and appropriate intervention).  

  • Maintains accurate and up to date records of visits and duties performed per Hospice documentation policies.  

  • Provides personal care (ie: bathing, oral hygiene, skin care, hair care, bath rules, changing of bed linen, checking of vital signs under supervision of primary nurse). 

  • Transfer of patient (ie: bed to chair, bed to commode, chair/commode to bed). 

  • Turning and positioning of patients. 

  • Assisting with bed pan/urinal. 

  • Assist with ambulation and exercise of patient as ordered and so assigned. 

  • Maintain clean, safe, and comfortable environment. 

  • Simple wound care, removal of impaction, and providing Fleet’s enema under RN supervision. 

Education:

  • High School Diploma or GED preferred.

Education:

  • Completion of approved nurse aide training program and successful completion of competency evaluation.
  • Current CPR certification

Experience:

  • At minimum, one year experience as a Nurse's aide.

Skills:

  • Sensitivity and openness to the physical needs/care of the terminally ill.
  • Maturity and ability to deal effectively with demands of the job.
  • Must have the ability to read, write, and carry out directions promptly and accurately.
  • Good physical and emotional health.

Other Requirements

  • Must have own transportation and provide proof of liability insurance which meets state requirements.
  • Must be able to travel to and from all patient homes without assistance.
  • Must be able to perform all aspects of the position unassisted.
  • Must have good communication skills and techniques with facilities, patients, families, physicians, and office staff.
