Duties Include
The Licensed Physical Therapist provides clinical care to patients’ in their home. Their duties include:

• Ensures quality and safe delivery of home health care services
• Following treatment plan
• Collaborates with physicians, other agency staff and contract services to coordinate and implement the Plan of Care
• Trains new clinicians by allowing clinicians to shadow patient visits
• Promotes education for clients, client families, and the community
• Completes all required documentation in accordance with the standards of the agency, and regulatory requirements

Job Requirement

• Possession of a Physical Therapist license valid for work in CA.
• Minimum 1 Year Home Health Experience is needed
• CPR/ALS Card certification.
• Possession of a valid Driver’s License or ID Card.
• Current Negative TB Test results.
• Current Physical Exam clearance.
• Documentation of COVID Vaccine Card or Exemption Proof.
• Availability to take Visits in Pleassanton, Walnut Creek, Freemont and nearby areas
