Work Environment

This role is primarily remote, yet you will collaborate with various community health organizations within the framework of a community health clinic. Key Responsibilities

• Conduct thorough assessments of clients’ social, emotional, and environmental challenges, ensuring a deep understanding of their unique circumstances.
• Develop and implement individualized service plans, outlining specific goals, interventions, and methods for monitoring progress toward those goals.
• Provide compassionate counseling to individuals and families facing adversity, employing techniques suitable for diverse socio-cultural backgrounds.
• Facilitate collaboration with clients in crafting personalized service plans, actively connecting them with essential community resources.
• Perform crisis interventions, risk assessments, and safety planning as situations demand, ensuring clients have immediate support during critical times.
• Build and maintain cooperative relationships with community agencies to coordinate services and advocate effectively for your clients’ needs.
• Participate in community outreach initiatives designed to enhance awareness of available mental health resources and promote our agency's services.
• Engage in regular team meetings, case conferences, and ongoing staff training to cultivate a well-informed approach to client management.

Qualifications

• A Master's degree in Social Work from an accredited institution.

Compensation and Benefits
